Submit this fee along with documentation of your three qualifying clients, and your Certification Application.

EXPLANATION OF ADDITIONAL FEES:

Late filing of your Certification documents (past the 2-year limit by no more than 6 months): You must ALSO pay the $50 late filing fee
If more than 6 months past your 2-year limit, you are considered "inactive." In order to become "active" again you must pay the $50 late fee, AND you must prepay and take a Refresher course. Reinstatement is not complete without the actual filing and approval of your forms.
